Dental Services – Patient Information

Dental services at Altitude Dental: we have available appointment times for regular dental exams, hygiene treatment, specific dental procedures or emergencies. If you are a new patient and this is your first visit to the dentist, please provide us with your dental care insurance and dental health care information (use our downloadable new patient forms).

Your initial appointment is based upon your expressed needs and can be discussed with one of our receptionists. If you require a specific or emergency exam, please advise our receptionist – if possible, treatment will be done the same day as the consultation.

Please assist us by providing the following information at the time of your consultation:

  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms (downloadable from our website). This will save time and allow us to help you process your claims.
  • If you are unable to download or complete the new patient form, then please arrive 10 minutes early for your appointment.

We provide dental services for Coquitlam, Port Coquitlam, Port Moody, Pitt Meadows, New Westminster, Burnaby, and many other municipalities in the Greater Vancouver area.

Important

Note: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (for example, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (for example, heart medications, aspirin, anti-coagulant therapy, etc.) or require medication prior to dental cleanings (for example, antibiotics for pre-medical services).

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. The standard of care requires that we have either a Panoramic X-ray (one that shows all the teeth and jaw bone) or x-rays that enable Dr. Vaida to diagnose the right treatment. Depending on whether or not the area being examined is on the supplied x-rays, we may need to take additional digital x-rays.

Health History

Please download and fill-out our New Patient form by downloading it from the Patient Forms section. After you have completed the form, please make sure to bring it on your first visit to our office. The security and privacy of your personal data is one of our primary concerns and we take every precaution to protect it.

If it is not convenient to download the form, please arrive about 10 minutes early for your appointment so that you can complete the required information at our office.
