Patient Records and Dental Forms
Coquitlam Altitude Dental
Patient records at our dental office are computerized for the most part. For your first visit to Coquitlam’s Altitude Dental, we need to gather information on your dental history and your dental health plan coverage.
We have the following forms that we require completed before your first appointment.
- New Patient Form – we require your medical and dental health history to ensure that we know and understand any underlying conditions and can respond appropriately
- New Patient Insurance Form – we require your dental health plan insurance information to ensure that the dental claims to your insurance provider are processed efficiently
Please download the above forms, complete them and either bring them into our office the day of your dental appointment, or before your appointment if you wish. You can also scan your forms and email them to our office at firstname.lastname@example.org.
If it is not convenient to download the forms, please arrive at our dental office about 10 minutes before your appointment so that you can complete the required information at that time.
Requests for appointments and treatment plans will be responded to within 1 to 2 business days with the exception of extended weekends or holiday hours – please contact us to book a dental appointment.
